ORDERING GRAFT
Orders for interstate and overseas hospitals should be submitted to PBTB by completing the Allograft order interstate overseas form.
PBTB requires Medical Staff to obtain the recipient’s written consent on our Form E Standard Recipient Consent before graft will be supplied. In addition to recording the patient’s awareness of the potential risks of transmission of disease via allograft implantation, the form enables surgeons to communicate their request for allograft to PBTB, provides details about the date and location of surgery, and the intended recipient’s details. Note that verbal consent will not be accepted.
We ask that surgeons advise us of the Rhesus status of female patients of child bearing age, and where possible Rhesus negative graft will be allocated for Rhesus negative patients.
Some surgeons prefer to use non irradiated femoral heads. A specific Form E.1 Consent Non Irradiated and a letter from the surgeon specifically requesting non irradiated tissue is required in such instances.
